How to Choose a Medicare Path

Compare the two principal coverage structures around care access, cost, travel, and flexibility.

First choose the coverage structure

Under Original Medicare, A and B provide federal coverage; you may add a separate Part D plan and, if eligible, Medigap. Advantage is an alternative private-plan way to receive A/B benefits and usually Part D. You need both A and B to join Advantage. Compare those systems before comparing logos or extra benefits: they differ in provider access, plan rules, premiums and out-of-pocket exposure.

Check every provider against the exact plan

Original Medicare generally allows use of any provider that accepts Medicare across the United States. Advantage routine care may depend on networks, referrals and prior authorization; some plan types permit particular out-of-network care at higher cost. Enter the actual physicians, facilities, specialists and ZIP code, then confirm current participation with both the plan and offices. A carrier name alone does not prove a particular contract is accepted.

Run a drug and pharmacy comparison

Enter drug names, strengths, dosages and pharmacies into Medicare’s plan comparison tool. Check tiers, deductible treatment, authorization and quantity restrictions. In 2026 covered Part D drugs have a $2,100 out-of-pocket cap, but that does not erase premiums or make a nonformulary drug covered. Compare the full year, not just the first monthly premium.

Model a routine and a high-use year

Add Part B, plan and optional Medigap premiums to realistic doctor, hospital and prescription spending. Original Medicare without supplemental coverage has no general annual Part A/B maximum; Advantage sets a yearly maximum for covered A/B care that can differ by network. Compare an expected-care year and a hospitalization year under the exact plan’s Evidence of Coverage.

Protect future options

Medigap purchase rights have timing rules and can depend on the state. Do not assume that leaving Advantage later guarantees a desired Medigap policy. Review enrollment windows, any qualifying trial or guaranteed-issue right, travel needs and annual plan changes before switching. If employer or retiree coverage is involved, ask its administrator what is lost by leaving it.
