Medicare Forms Library
Identify common federal forms and understand when they may be relevant.
Start with Medicare’s official form finder
The federal Medicare forms library groups enrollment, claims, appeal and other forms by the action you need. Use the current form and instructions from Medicare.gov instead of an undated copy on another website.

Part B after employer coverage
Medicare lists form CMS-40B for applying for Part B and CMS-L564 for employment information in relevant Special Enrollment Period situations. SSA handles enrollment; check its Part-B-only application instructions and the exact evidence required.

Income-related premium reconsideration
For a qualifying life-changing event affecting IRMAA, SSA describes the request and form SSA-44. A loss of income alone is not a promise of approval; use SSA’s event criteria and supporting documentation.

Claims and appeals
A coverage decision notice identifies the action and timeframe to dispute it. Medicare’s appeal forms and guidance differ between Original Medicare, private health plans and Part D. Keep the notice rather than picking a generic appeal form.
