What Part D Covers

Learn how formularies, protected classes, tiers, and plan rules affect prescription access.

A formulary is the starting point

Medicare Part D plans cover a broad range of outpatient prescription drugs, including most medications in certain protected classes. That does not mean every drug, brand or strength is on every plan’s formulary. Search the exact drug in the current plan year and confirm it with the plan, especially if a doctor has prescribed a particular formulation.

Rules that affect access

A covered drug may still require prior authorization, step therapy or quantity limits. If the doctor considers an alternative inappropriate, an exception can be requested for a nonformulary drug or a coverage rule. The plan makes a coverage determination, and appeal rights apply to adverse decisions. Check the exact paperwork and whether a temporary supply applies during a transition.

Part B and Part D are different

Some medicines are covered under Part B because of how and where they are administered, such as certain drugs used with covered durable medical equipment. Part D generally addresses covered outpatient prescriptions not paid under Part B. The coverage path can change the applicable deductible or cost sharing. Ask the provider how a specific drug will be billed before assuming it falls under Part D’s cap.

Pharmacy choice changes the result

Part D plans can use preferred and standard in-network pharmacies and may charge more, or not cover certain fills, outside the network. Include your preferred local and mail-order pharmacies in a yearly cost comparison. A prescription appearing on the formulary does not prove your chosen pharmacy has preferred pricing or the needed drug in stock.

Price the complete year

The 2026 covered Part D out-of-pocket cap is $2,100; it does not make noncovered medicines free or replace the monthly premium. Enter the precise medication list into Medicare’s comparison tool and review the plan’s restrictions. If income and resources are limited, SSA’s Extra Help application may reduce eligible Part D expenses.
