Medicare Costs
Build a complete annual cost picture instead of comparing premiums alone.
Part A in 2026
Most people pay no monthly Part A premium. A person required to buy it may pay $311 or $565 a month, depending on Medicare-tax history. Under Original Medicare the hospital inpatient deductible is $1,736 per benefit period, not once per calendar year. After it, daily coinsurance begins on hospital day 61: $434 for days 61–90 and $868 for each lifetime reserve day. Skilled nursing has its own daily charges after the first 20 covered days.

Part B in 2026
The standard Part B premium for most beneficiaries is $202.90 a month. Original Medicare’s annual Part B deductible is $283; after it, many covered services leave 20% of the Medicare-approved charge to the beneficiary when the provider accepts assignment. Income-related surcharges and late penalties can increase the premium. A Medicare Advantage plan may use a different deductible and cost-sharing schedule for its members.

Prescription costs in 2026
Covered Part D drug spending has an annual out-of-pocket limit of $2,100 in 2026. Each plan still has a premium, formulary, pharmacy contracts and rules that affect total spending. The cap is for covered Part D drugs, not general medical care, noncovered drugs or monthly premiums. The optional Prescription Payment Plan changes the timing of covered-drug payments but does not reduce total costs.

Compare a whole year, not an advertised premium
Add all monthly premiums to likely medical and drug copays, coinsurance and deductibles, then model a higher-use year. Original Medicare has no general annual Part A/B maximum without supplemental coverage. Advantage plans set a yearly maximum for covered A/B services, potentially with different in-network and out-of-network limits. Medigap premiums and standardized benefits form a different budget than Advantage.

Check assistance and actual offers
Medicare describes programs that may help people with limited income and resources pay premiums and drug costs; SSA administers Extra Help applications. The exact local plan’s Evidence of Coverage, formulary and provider list supply real premium and cost-sharing figures. The national base Part D premium used in penalty calculations is not a quote for an individual plan.
